TRANSITIONS provides comprehensive support through life’s transitions, focusing on promoting self-sufficiency, health, and social confidence.

We offer three Day Treatment and Training Adult (DTA) programs in Tempe, AZ, and Glendale, AZ, which focus on fun, responsibility, and building essential life skills. Additionally, we provide Home & Community-Based Services (HCBS), including Respite, Habilitation, and Attendant Care, offering temporary relief and one-on-one training for individuals with developmental disabilities. Our services also encompass physical, mental, and emotional support to ensure a safe and healthy living condition for everyone.
