The Community Healthcare Network (CHN) is a federally-qualified community health center dedicated to providing quality healthcare services for the whole family across the Bronx and Queens. Since 1981, CHN has served the community by offering comprehensive care, ensuring no one is turned away.

CHN offers convenient services, including scheduling appointments, checking lab results, and accessing telehealth consultations with doctors, nurses, and medical experts. Our centers provide diverse services, from primary care and school physicals to specialized care, including transgender health and sexual health services. We are open Monday through Friday, 9 am – 5 pm, and serve locations throughout areas like East New York, Long Island City, and the Lower East Side.
