Project Transitions: Ending the HIV Epidemic through Supportive Housing and Care

Project Transitions is dedicated to ensuring every person living with HIV and AIDS receives compassionate care, housing, and comprehensive support in Central Texas. We address the significant service gap by providing supportive housing, hospice, and recuperative care to those often left behind, including individuals facing health crises.

Currently, there are only 30 dedicated housing units for people living with HIV in the Austin area. We are actively building new, affordable housing to provide stable environments and wrap-around services that ensure medication adherence and help bring more people to undetectable + untransmittable status. Since 1988, we have provided essential care, supported by the generosity of our community, working toward an AIDS-free Austin.
